Summary
Regional and global food systems are constantly evolving, thus the contextual elements presented in Chapter 1 are likely to evolve, and food systems will be transformed. Because it is impossible to predict the food systems of tomorrow, we have adopted a foresight approach in order to try and understand possible future changes. Our approach, which is presented in the first part of this chapter, has therefore mainly been based on identifying the main drivers of the transformation of food systems. This work was the fruit of collective discussions by a multidisciplinary group made up of some 15 experts. The plurality of their views and their areas of competence allowed them to analyse the potential impacts of the different evolutions identified relative to the sustainability of food systems in terms of their nutritional, economic, social, cultural, environmental and territorial dimensions. This step of the analysis also enabled us to highlight a certain number of points at issue, which are presented in the second part of the chapter; this does not end with a presentation of different scenarios (as might have been expected), but concludes with the three transversal messages arising from debate by this workshop: issues linked to inequalities of access to food, territorial dynamics and the governance of food systems.
Food systems evolving under the effects of various factors
Through the identification of factors underlying the transformation of food systems, it appears clearly that some trends have already been identified (see, in particular, Chapter 1 on the context and the challenges of food systems, and the retrospective analysis described in Chapter 2). Nevertheless, these trends involve a certain number of questions and uncertainties, notably regarding the nature and degree of their potential effects. These uncertainties thus open the way to contrasting scenarios for food systems throughout the world (see Figure 9.1).

Care-related infections are a major public health concern. Their transmission can be associated with environmental factors. This study looks at air contamination around 45 patients colonized with multidrug-resistant organisms (MDROs). We found that 30 hospital rooms (67%) were contaminated with MDRO species and 10 rooms (22%) were contaminated with at least 1 MDRO.

Estimating Attributable Mortality Due to Nosocomial Infections Acquired in Intensive Care Units

Background.
The strength of the association between intensive care unit (ICU)-acquired nosocomial infections (NIs) and mortality might differ according to the methodological approach taken.
Objective.TO assess the association between ICU-acquired NIs and mortality using the concept of population-attributable fraction (PAF) for patient deaths caused by ICU-acquired NIs in a large cohort of critically ill patients.
Setting.Eleven ICUs of a French university hospital.
Design.We analyzed surveillance data on ICU-acquired NIs collected prospectively during the period from 1995 through 2003. The primary outcome was mortality from ICU-acquired NI stratified by site of infection. A matched-pair, case-control study was performed. Each patient who died before ICU discharge was defined as a case patient, and each patient who survived to ICU discharge was denned as a control patient. The PAF was calculated after adjustment for confounders by use of conditional logistic regression analysis.
Results.Among 8,068 ICU patients, a total of 1,725 deceased patients were successfully matched with 1,725 control Patients. The adjusted PAF due to ICU-acquired NI for patients who died before ICU discharge was 14.6% (95% confidence interval [CI], 14.4%—14.8%). Stratified by the type of infection, the PAF was 6.1% (95% CI, 5.7%–6.5%) for pulmonary infection, 3.2% (95% CI, 2.8%–3.5%) for central venous catheter infection, 1.7% (95% CI, 0.9%–2.5%) for bloodstream infection, and 0.0% (95% CI, –0.4% to 0.4%) for urinary tract infection.
Conclusions.ICU-acquired NI had an important effect on mortality. However, the statistical association between ICU-acquired NI and mortality tended to be less pronounced in findings based on the PAF than in study findings based on estimates of relative risk. Therefore, the choice of methods does matter when the burden of NI needs to be assessed.

In this paper, we present CVD (Chemical Vapor Deposition) growth and passivation of tungsten (W) and titanium nitride (TiN) nanocrystals (NCs) on silicon dioxide and silicon nitride for use as charge trapping layer in floating gate memory devices. NCs are deposited in an 8 inches industrial CVD Centura tool. W and TiN are chosen for being compatible with MOSFET memory fabrication process. For protecting NCs from oxidation, a silicon shell is selectively deposited on them. Moreover, for a better passivation, TiN NCs are encapsulated in silicon nitride (Si3N4) in order to get rid of oxidation issues. After high temperature annealing (1050°C under N2 during 1 minute) XPS measurements point out that NCs are still metallic, which makes them good candidates for being used as charge trapping layer in floating gate memories.

Alors que l'alimentation de l'homme a beaucoup évoluée depuis ses origines, le génome humain est resté très stable. Pourtant, de très nombreux gènes ont des polymorphismes connus. En fait, on considère maintenant que les principales pathologies humaines (maladies cardiovasculaires, diabète, obésité et cancers) résultent d'une interaction entre des facteurs de succeptibilité génétique et des facteurs de l'environement, dont l'alimentation. Dans le domaine du métabolisme des lipoprotéines et des maladies cardiovasculaires, des polymorphimes de plusieurs gènes ont été identifiés et associés aux niveaux des paramètres lipidiques ou aà des réponses variables aux régimes, comme pour les apoprotéines (apo) E, B, A-IV et C-III, le LDL récepteur, la protéine microsomiale de transport (MTP), la protéine de liason des acides gras (FABP), la protéine de transport des esters de cholésterol (CETP), la lipoprotéine lipase ou la lipase hepatique. Metal Transport and Loss in Hafnium and Lanthanum Aluminate Films on Si Hampered by Thermal Nitridation

The effects on metal transport and loss in Hf and La aluminate films deposited on Si induced by rapid thermal annealing at 1000°C were investigated. Decomposition of HfAlO films on Si during rapid thermal annealing was reveled by the decrease of the Hf and Al contents. Metal loss from LaAlO/Si structures was also observed following annealing in vacuum, while strong metal transport and interfacial reaction were induced by annealing in a O2 containing atmosphere. These instabilities were hampered by means of post deposition thermal nitridation in NH3 at temperatures lower than 1000°C performed before the rapid thermal annealing step. The role of nitridation is discussed in terms of the N profiles in the nitrided structures.

The immunosuppressant drug cyclosporin A (CsA) inhibits T-cell function by blocking the phosphatase activity of calcineurin. This effect is mediated by formation of a complex between the drug and cyclophilin (CyP), which creates a composite surface able to make high-affinity contacts with calcineurin. In vitro, the CyPB/CsA complex is more effective in inhibiting calcineurin than the CyPA/CsA and CyPC/CsA complexes, pointing to fine structural differences in the calcineurin-binding region. To delineate the calcineurin-binding region of CyPB, we mutated several amino acids, located in two loops corresponding to CyPA regions known to be involved, as follows: R76A, G77H, D155R, and D158R. Compared to wild-type CyPB, the G77H, D155R, and D158R mutants had intact isomerase and CsA-binding activities, indicating that no major conformational changes had taken place. When complexed to CsA, they all displayed only reduced affinity for calcineurin and much decreased inhibition of calcineurin phosphatase activity. These results strongly suggest that the three amino acids G77, D155, and D158 are directly involved in the interaction of CyPB/CsA with calcineurin, in agreement with their exposed position. The G77, D155, and D158 residues are not maintained in CyPA and might therefore account for the higher affinity of the CyPB/CsA complex for calcineurin.
